Urine ACR measures albumin relative to creatinine. A routine urine examination looks at a broader set of urine characteristics; neither name automatically includes the other.
What the ACR measures
The albumin-creatinine ratio compares the amount of albumin with creatinine in a urine sample. It helps assess albumin loss while accounting for urine concentration. The result is used with other information when evaluating kidney health.
Copy the ratio and its units, not just an albumin number, when recording the report. Laboratories may use different unit conventions. A value in mg/g should not be compared directly with the same number written in mg/mmol.
What routine urine examination adds
Urine routine examination may include physical, chemical and microscopic findings. The exact components vary. A urine culture is another investigation, used to assess microbial growth when indicated; it is not automatically part of either a routine examination or an ACR.
Ask the booking team to match the precise prescription. “Urine test” is too vague when selecting a package. This matters if your clinician specifically requested ACR, microscopy or culture, since substituting a differently named test can leave the intended question unanswered.
Why a positive result may need confirmation
A clinician may repeat an elevated urine albumin result and consider the circumstances around collection. Exercise, acute illness and other temporary factors can complicate interpretation. One result should not be used to label a permanent condition without assessment.
Keep the collection date and any preparation or contamination issue with the report. Tell the clinician about symptoms and relevant medicines. Do not try to correct the number by extreme water intake or by changing treatment without advice.
Getting the sample right
Use the container and instructions supplied for the ordered test. Ask whether a first-morning or other sample is preferred, how to collect it and how quickly it should reach the laboratory. Instructions can differ between an ACR and a culture.
If a sample spills, is contaminated or cannot be delivered as instructed, tell the team before it is processed. A correctly identified, properly collected sample is more useful than a technically complete report built on unsuitable material.
Common questions
Is microalbumin a different protein?
The term is used in albumin testing; ask whether the order is for albumin concentration or albumin-creatinine ratio, because the reported measurements differ.
Can a routine urine report replace ACR?
Not when ACR is the specifically requested measurement. Confirm the named test.
Does one high ACR confirm permanent kidney damage?
No. The result needs clinical context and may need confirmation.
